The referral process

A straightforward referral process makes it easy to refer people to us, while detailed assessments promote the best outcome:

  1. The referral process begins with a referral form, which can be downloaded from our website or requested by phone or email. The referral form asks for basic information about the individual, including their name, age, gender, and contact details, as well as their medical and support needs.
  2. Once we receive the referral form, we will contact the referrer to discuss the person’s requirements in more detail. We may also request additional information, such as medical reports, existing care plans, and risk assessments, to ensure that we have a complete understanding of the individual’s needs.
  3. When we have all the necessary information, we’ll conduct an initial assessment to determine whether we can provide the appropriate level of care and support to meet their needs. If we determine that we are able to provide the required level of care and support, we will work with the social worker or commissioner to develop an individualised support plan.
  4. The support plan will be tailored to the specific needs and goals of the person and will include details about the level of support required, the frequency of support, and the duration of the support. We will also discuss any specific requirements or preferences they may have, such as cultural or religious considerations.
  5. Once the support plan has been agreed upon, we will assign a dedicated support worker to the person, sometimes referred to as a key worker. This support worker will be responsible for implementing the support plan and providing ongoing support and care. We will also provide regular updates to the social worker or commissioner to ensure that they are informed of their client’s progress and any changes to their needs.
